We issue two certificates — one for the clinical drug & alcohol evaluation, one for completion of the DV-MRT program. Both are printed on watermarked stock, hand-embossed with the clinic seal, and shipped directly to the attorney or probation officer you designate.
Drug & Alcohol Evaluation Certification
Issued on completion of the $165 flat-rate evaluation. Confirms the clinical interview was conducted, the screening instruments were administered, and a signed diagnostic report was produced.
